After making an appointment with your counselor, you are asked to fill out the paperwork (linked below) and bring it with you to your first visit. Be sure to bring your insurance card, if applicable, or Employee Assistance Program (EAP) authorization number, as well as any deductible or co-payment fee. We accept cash, checks, debit and credit cards. Have a seat in our inviting welcome area and your counselor will be out to take you to his or her office.

Our independently licensed therapists (over 6,000 hours of experience) charge $175 for the first session (1 hour and 20 minutes) and $125 for each additional 50 minute session. Our licensed therapists (over 3,000 hours of experience) charge $150 for the first session (1 hour and 20 minutes) and $100 for each additional 50 minute session.  Our licensed counselor trainees charge $125 for the first session (1 hour and 20 minutes) and $75 for each additional 50 minute session. Many insurance companies, as well as employee assistance programs, will help to cover the cost. If cost is an issue, your counselor will be happy to discuss payment plans, or reduced cost services.

Children are not permitted to be left in the welcome area alone. If your child(ren) are not scheduled for an appointment, we ask that you leave them with a childcare provider in an alternative location, so that we might best serve our clients, including you.

Minors will need a parent or guardian present at the first session to fill out authorizations for treatment. Your counselor will instruct you as to their desire for your presence at the sessions that follow.

**There is a fee of $65 for any appointments that are cancelled within less than 24 hours for a non-emergency and for any sessions that are not attended without a previous made phone call (no call no show).

This form is required for all clients.  ONLY your counselor will have access to this information, it will be used to cover any balances that incur from insurance deductibles, no call no show fees, or even your copay or session fee if you would like.  You will always receive receipts from your counselor for any payments that were run.

Insurance/EAP paperwork: Please fill out and bring with you to your session if applicable: Do not rely on your insurance card to tell you your copay.  Often insurance companies have a different copay and deductible for behavioral health.
